The following insurance-related administrative actions were posted in the Friday, September 12, 2008, edition of Florida Administrative Weekly.
Notice of Proposed Rule: Division of Workers’ Compensation Forms and Instructions
The Department of Financial Services, Division of Workers’ Compensation, has issued a Notice of Proposed Rule for the following Rule:
- 69L-6.009: Forms and Instructions
The purpose of this Rule is to revise forms to eliminate the collection of social security numbers in order to comply with Section 119.071(5), F.S. (2007), which precludes an agency from requesting an individual’s social security number unless such collection conforms to its specified provisions. As such, the proposed Rule would revise the following forms:
- Form DWC 250 (Notice of Election to be Exempt), is amended to include a purpose and use statement regarding the collection of the social security number
- Form DWC 250-R (Notice of Revocation of Election to be Exempt), DWC 251 (Notice of Election of Coverage), and DWC 251-R (Revocation of Election of Coverage) are amended so that the collection of the social security number is discontinued.
If requested within 21 days of the date of this notice (by Friday, October 3), a Hearing will be held on October 8, 2008 at 10:00 a.m. in Room 104J of the Hartman Building, 2012 Capital Circle S.E., Tallahassee, Florida.
