The proposed Rules would establish required procedures for submitting documentation through the DFS online Education Database, which can be accessed at www.myfloridacfo.com.
If requested in writing and not deemed unnecessary by the agency head, a Rule Development Workshop will be held at the date, time and place below:

Notice of Rulemaking Development: Workers’ Compensation Form Revisions
The Florida Department of Financial Services, Division of Workers’ Compensation, has issued a Notice of Development of Rulemaking regarding the following Rules:
- 69L-10.006: Definitions
- 69L-10.012: Review of Proof of Claim
- 69L-10.015: Deemed Denial – 120 Day Time Period
- 69L-10.016: Documenting Expenditures for Purposes of Obtaining Reimbursement
- 69L-10.017: Reimbursement to Subsequent Employer
- 69L-10.019: Forms
The proposed Rules would revise forms to comply with the section of Florida law that precludes an agency from collecting an individual’s social security number unless such collection conforms to specified provisions of that statute.
The following forms would be revised:
- Forms SDF-1 (Proof of Claim), and SDF-2 (Reimbursement Request) would be renumbered, and amended so that the collection of the social security number is discontinued
- Form SDF-6 (Explanation of Benefits) would be renumbered and amended so that the collection of the social security number is discontinued
In lieu of social security numbers, the revised forms would utilize an “SSDTF Claim Number” for purposes of identifying and tracking claims.

Notice of Proposed Rule: Examination of Insurers
The Florida Department of Financial Services, Office of Insurance Regulation, has issued a Notice of Proposed Rule regarding the following Rule:
- 69O-138.005: Examination of Insurers
The purpose of the proposed Rule is to set forth procedures for examinations of insurers performed by specialists contracted by the OIR.
This proposed Rule details criteria for the specialists to qualify to conduct the examinations, application process, and compensation information.
If requested by Friday, October 24, 2008, a Rule Development Workshop will be held at the date, time and place below:

Notice of Proposed Rule: Reserve Liabilities for Pre-need Life Insurance
The Florida Department of Financial Services, Office of Insurance Regulation, has issued a Notice of Proposed Rule regarding the following Rule:
- 69O-164.040: Determining Reserve Liabilities for Pre-Need Life Insurance
The purpose of the proposed Rule is to recognize the inadequacy of the 2001 Commissioners’ Standard Ordinary Life Valuation Mortality Table (“2001 CSO”) for use in determining the minimum standards of valuation and nonforfeiture value, as well as to require continued use of the 1980 Commissioners’ Standard Ordinary Life Valuation Mortality Table (“1980 CSO”) determining the aforementioned minimums.
The proposed Rule allows use of the 2001 CSO for pre-need life insurance policies issued before January 1, 2012, but only after the insurance company submits documentation demonstrating they have adequate reserves.
