Florida Division of Workers’ Compensation Rule Hearing
The Florida Department of Financial Services, Division of Workers’ Compensation, has issued a Notice of Proposed Rule regarding the following Rule:
- 69L-6.009: Forms and Instructions
The purpose of the proposed Rule is to revise the following forms to comply with the Florida law precluding an agency from requesting an individual’s social security number unless such collection conforms to the specified provisions of that statute. The proposed Rule would revise the following forms:
- Form DWC 250 (Notice of Election to be Exempt), would be amended to include a purpose and use statement regarding the collection of the social security number
- Forms DWC 250-R (Notice of Revocation of Election to be Exempt), DWC 251 (Notice of Election of Coverage), and DWC 251-R (Revocation of Election of Coverage), would be amended so that collection of the social security number is discontinued
The proposed Rule amendment also would revise the existing field office address.
