Comments Requested: PIP Provider Certification of Eligibility Form

 

The Florida Office of Insurance Regulation (“OIR”) has scheduled a workshop for November 6, 2007 to discuss Proposed Rule 69O-170.0155(n), relating to the Personal Injury Protection (“PIP”) Provider Certification of Eligibility Form (“Form”).

A member of this Firm will be attending the workshop.  We have attached the proposed draft of the Form for your review and accordingly request that you provide us with your comments as soon as possible.
